    I have a patient who is on high dose B12 and 5MTHF and still having methyltion issues. Mainly due to CBS + status resulting is high levels of Taurine and Ammmonia. Would in this case TMG be the preferred suppliment to help offset this?

        Dr. Hartman,
        Because of the elevated ammonia and taurine, which definitely could be coming the gut dysbiosis, I would also add some activated charcoal (AC) – one to two capsules daily (what for constipation. Can add some magnesium oxide if needed), and alpha-ketoglutate (AKG) – 300mg daily. The AC will help to bind up gut toxins, and the AKG will help to process ammonia.

        What is the purpose of the AKG? How does it help process NH4?

          One of its effects is to complex amino groups and deliver through the urea cycle.

          P.S. it is a double-edge sword though in that it is also involved in the glutamate production which should then be converted to GABA.
