      Good morning.

      I have a 12 year old diagnosed with ADHD for some 3 years now. Her mom says her daughter exhibits anger and obsitinance. She is currently on Adderall 10mg and has tried omegas and L-theanine and melatonin WITHOUT HTP

      Before I send her mom the results I wanted to get your thoughts on this protocol (i did get the assistance of the 1:1 from GPL but there are still things I don’t understand. I will provide you a brief summary and questions to follow:
      Arabinose elevated to 73
      HVA/ DOPAC Ratio low 5-1.4
      Hydroxyindoleacetic (5-HIAA) High 8.4
      deficient in B6.

      I don’t understand what it means if the HVA DOPAC ratio is low. Perhaps I misunderstood but I was told that it is because neurotransmitters are building up. How is it being built up if the ratio is low? Magnesium, methylated B complex and SamE was suggested. What exactly does SamE. Im just a little overwhelmed.
      5-Hydroxyindoleacetic (5-HIAA). Could the reason this be high is due to potential celiac? She is not eating high amount of serotonin foods and her melatonin does not contain HTP

      She also has mycotoxins.

      Could all these symptoms be caused by the mold?

        DOPAC is a side pathway for dopamine. Preferentially, dopamine converts to norepinephrine (NE) via dopamine beta-hydroxylase (DBH). But, there will be some dopamine that gets peeled off towards DOPAC. From there, DOPAC converts to HVA. This conversion to HVA from DOPAC requires the enzyme COMT and the methylation cycle component SAMe (S-adenosylmethionine). SAME is the major methyl donor in the body. It too is responsible for assisting in the conversion of NE to epinephrine. A low HVA to DOPAC ratio would indicate a either a normal or high DOPAC, but low conversion to HVA. If you look at the chart on page #2 of the OAT you can see the flow of what I am describing. A low HVA/DOPAC ratio means there is a build-up of DOPAC that cannot find its way to HVA adequately. There is something interfering with the step from DOPAC to HVA.

        SAMe supplementation is necessary to make the conversion happen. It is possible that SAMe is just not being produced in adequate supply. The production of SAMe requires magnesium. The methylated B complex provides overall cofactor support for the methylation cycle. Therefore, what GPL representative recommended is spot on. Unfortunately, there is no exact dosing amount for SAMe for age or weight. Pure Encapsulation carries a SAMe product that is 200mg daily. The general dosing range is broad between 200mg to 600mg twice daily. I would suggest being conservative and keep your initial dose to 200mg to 400mg daily. For magnesium in these cases of anger, aggression, etc. I like magnesium L-threonate.

        The 5-HIAA could be from Celiac if she is not on a GFCF diet. You would have to confirm it through testing.

        The connection to Mycotoxins is a possibility, but I would try and avoid at this time connecting all problems to the existence of mycotoxins. Yes, the mycotoxins are problematic, but I cannot say with certainly that all of her issues are 100% linked to the existence of mycotoxins. Much of this will get flushed out in time as you start implementing more interventions like the list your provided.

          Is there a methylated B complex (kids) you could recommend and not just from New Beginnings? Shipping to Canada is so expensive so I would like other alternatives.
          Also if this child also needs B 6 would the B complex not include that?
