      Parents brought in a 10 year old boy whose main complaints are fatigue, stomach aches, and headaches. His parents have done all kinds of remedies and ‘self-treating’ for 2 years (including colonics, diet counseling, homeopathy, etc, etc, etc) and they feel nothing has helped him. The first testing he has ever done was through me.
      His GI panel showed some bacterial imbalances (no infections), slightly elevated lactoferrin (9), and elevated sIGA (500). I would tend to believe these are diet related.
      He did have minimal food sensitivities and has been eliminating them.
      Finally, Organic Acids shows: minimal Arabinose (97), oxalic acid at 250, high Quinolinic/5-HIAA ratio (4), high B5 (32), very low vitamin C.
      All else normal.
      Recent email from parents (very lengthy) states that he is not doing well.
      Did discontinue the multi-vitamin because it ‘gave insomnia’, says he just lays around all day and can only crawl because of fatigue, has had more episodes of rage and depression and is random. Hits himself in the head, wants to die. This is NOT normal for him. Episodes can last 20 minutes to an hour, and once per day or skip a few days between episodes.
      Asked more practitioners for more testing, thyroid is normal. Parents are worried about yeast and adrenal fatigue and convinced that the labs don’t show that he may have more yeast. (I feel they are obsessed with the thought of yeast overgrowth). Now they are worried about endocrine problems and want a referral out to specialists for this testing as well.

      Can starting to address his diet and minor yeast growth cause this much of an effect? I have a feeling that there are more psychological things going on since these labs are not particularly out of range or concerning.

      I talked to Doc about this and how I feel problems are seen as even bigger problems. Start a new diet, new problem arises. Start a nutritional medicine, new problem arises.
      Interesting to discuss especially on the lab markers I gave.

        Dr. Watkins,
        I would agree with you regarding the slightly high lactoferrin and sIgA. The arabinose indicating yeast could cause too, along with the oxalic acid.

        I have seen 100’s of similar looking labs without this level of problems you are describing. A family dynamic and/or psychological issues could certainly be at play. However, I would still offer up some recommendations:

        -low oxalate diet – great plains laboratory has a good handout through their article/brochure section on the low oxalate diet. Also, http://www.lowoxalate.com is helpful too.

        -start him on some chewable calcium/magnesium tablets with each meal and VSL#3 probiotic – two capsules daily.

        Also, I would strongly recommend doing a Hair Analysis from Great Plains, as well as have his Total Cholesterol checked.

        -There doesn’t appear to be any mitochondrial markers out of balance so I seriously doubt this is a real problem.

        -Adrenal fatigue could be a contributing factor so doing a Functional Adrenal Stress Profile is worthwhile. Not sure if you have a lab you use, but my preference is BioHealth Laboratory.

        -Finally, I would personally tackle the yeast as a trial to see if it helps. In this circumstance my tendency would be to do some Diflucan for 30 days to see what clinical effect can be seen. Usually, Diflucan gives a rather quick effect and can be useful as a ‘treatment’ trial.
