HPI: 10 yo girl. Mild anxiety in past. Past summer- rash on back. S/p peds appts- no tx.  After that developed severe OCD, moodiness, sensory issues, anxiety in school. CBT x 6 wks. Dx with PANDAS after mom learned about this. S/p Pandas specialist in Grove City. Started Antibiotics- ceftinir , azithromycin, then augmentin and biaxin. Then added magnesium and omega 3. OCD got worse in past few weeks. Saw ND- thought bartonella so gave homeopathic tx. Tingling in neck, stomach, hands. Leg cramps. No decrease in anxiety/OCD. +visible bloating. BM daily.  +pellets. In woods a lot. High tic area. No known tick bite.+ Stomach aches, HA.
Mom thought sx worsened on abx.
IgG food test- mostly just Egg
Labs – ASO AB- 352 DNASE-B 609 Mycoplasma- IgG high IgM high. vit D- 26. EBV IgG ab- 3.32-high,
IGeneX – +31 and +41 (positive for IGeneX standards)
I’m treating her mild yeast from the OAT.
Clinically sounds like could be Pandas or Lyme. She also had a cat scratch and we’re waiting for bartonella results.
Where do I go from here. Revisit antibiotics but maybe a different one?
Such a sweet, smart girl but this new anxiety is debilitating for her.

Here are her IGeneX results- this is the first time I’ve used this lab- not sure how to interpret this!

      Jessica,
There is not a lot on the OAT that points to the significance of this young girls problems. Treating for yeast makes sense, but I doubt its a major factor overall. The low phosphoric correlates with the low Vitamin D. Definitely work to get that up.

I would suggest running an Hair Metals test to check for metals like Aluminum, Lead and Mercury. But, sounds like you are dealing with mostly a Lyme/co-infection and PANDAS problem. Tough situation as the autoantibody reactions to these pathogens, particularly strep, cause OCD/anxiety.

I am not a Lyme or co-infection expert. Sounds like you have done some Lyme work?

In these types of cases where there is so much going on I am finding that the ION Cleanse from AMD is helpful – http://amajordifference.com. It’s not a specific treatment for Lyme, but it helps regulate the ion gated channels in the brain and nervous system so neurochemical balance is restored. It also aides in detoxification. You will likely still need to pursue antimicrobial treatment, but the ION cleanse opens up the pathways to function better.

From an autoimmune standpoint, I would also advocate for Low Dose Naltrexone – http://lowdosenaltrexone.org.

We are going to be talking about both of these interventions later in the course too.

I don’t know about Biocidin specific for Lyme, but their products are very good and I would suggest contacting BioBotanical Research directly. I am forgetting the name of the main person there who heads up their scientific/clinical research, but if you call the main number, tell them you were referred by me and wish to speak to someone about a clinical case they should get you in fairly quickly.

      I hope this helps.

      Sincerely,
      Dr. Woeller
